Well I have my sewing table of course then I put a card table in front of the sewing table and my ironing board on the left side of my sewing table and card table. I am doing a king size right now and it is working ok. I sometimes have to put it over my shoulder also but I can't do that for too long because of some pain I get in my neck (real pain not a person lOL) .

I've been starting in the middle and work out to the sides...stitch in the ditch so far. I folded the quilt up on the heavy big side and either lay it accordian style in my lap to quilt it or over my shoulder and start feeding it to the machine. It is heavy though and have to take breaks. I may put a table on my left to start laying the quilt on that if I continue trying to machine quilt...A LA quilting machine would be nice to own if I do make a lot of quilts.

I use a walking foot. It really helps to keep the fabrid from slipping. I only use it for sewing the finishing sides of my quilts.
I always hand quilt (stitch-in-the-ditch)
