What is the common term to use on a label when it is completely made by one person.

I think you can just say "Made and Quilted by: and the name".

You can be as detailed as you like. Your name as the quilter, the name of the pattern, the date.

some even make a pocket on the back and include a typed detail of pattern and a story about the quilt that can be saved for future owner of the quilt. - The Quilt's Story.
