Broken Thread

I have a Gammill Vision long arm. I have having trouble with the thread at the needle being so tight that it breaks. I have even bent a needle. I have loosed the tension to the point of the knob coming off--with no results. Any suggestions?

My DH is the long-arm operator in our house and he says first, re-thread the machine and change the needle. it sounds to him like your thread path is interupted. Or maybe your thread has gotten wound around the spindle at the spool.
